- Your responses on the downloadable wire transfer form below must be typed (incomplete or handwritten forms can’t be processed); please complete the form before printing
- Do not sign the form until you can be verified in branch or by a licensed notary
- For faxed requests, a valid notary’s signature and visible stamp are required
- Domestic wire transfers to U.S. locations usually reach their destination in 1 to 2 business days. International wire transfers may take longer.

Wire transfer form overview:
- Outgoing domestic wire transfers*: To send a wire transfer to another financial institution domestically, please complete the Domestic Wire Request Form Found here. To process the transfer, your responses must be typed.
- Outgoing international wire transfers**: To send a wire transfer to another financial institution internationally, please complete the International Wire Request Form Found Here. To process the transfer, your responses must be typed.
Please return all completed pages of the wire transfer forms to any Elevations Credit Union branch or fax to (303) 247-8835, Attention: Payment Services. To confirm receipt of your faxed wire request, please contact us at (800) 429-7626.
*A Member Service Representative may call to verify any wire request. A fee of $20.00 will be charged for Outgoing Domestic Wire Transfers per our Fee Schedule.
**Fees for Outgoing International Wire Transfers are $45.
